Healthy Families

OUR MISSION IS TO EMPOWER FAMILIES SO THEIR GOALS CAN BE DEFINED AND MET AND PARENTS CAN BECOME THEIR CHILDREN'S FIRST AND BEST TEACHERS.

WHAT IS HEALTHY FAMILIES OSCEOLA/SUCCESS-BY-SIX?

Healthy Families Osceola/SBS is a voluntary, intensive home visiting program designed to help new parents give their children a healthy start.

Healthy Families Osceola/SBS promotes positive parenting and child growth & development with individualized family plans designed by the family with help and guidance from their Family Support Worker.

Home visits focus on parenting skills, child development, child health and other aspects of family functioning and can continue through the child's fifth year.  

Healthy Families Osceola/SBS works with other community organizations to provide an array of services needed by participating families.

HEALTHY FAMILIES OSCEOLA/SUCCESS-BY-SIX GOALS:

Target first-time, teen and overburdened parents and enhance their ability to create stable, nurturing home environments.

Promote child health and development.

Build positive parent-child interaction skills.

Help ensure that families' social and medical needs are met.

Foster independence and encourage families to becoming actively involved in our community. 

Ensure that families are satisfied with the program and the services they receive.

HOW DOES A FAMILY ENTER THE HEALTHY FAMILIES OSCEOLA/SUCCESS-BY-SIX PROGRAM?

PREGNANT WOMEN AND FAMILIES OF NEWBORNS UP TO 2 WEEKS OLD ARE ACCEPTED, IF ELIGIBLE.

A pregnant woman can be referred to screening by Healthy Start, other programs which work with pregnant women, and by self-referral.

Newborns are screened at the hospital following delivery or may be referred during the first two weeks of life.

Live in targeted zip codes.

Pregnant teens/teen parents who live anywhere in the county may be eligible.

If the screening is positive and the client desires an assessment for eligibility, it is scheduled.

The program is entirely voluntary.
