To Obtain Birth and Death Certificates:
You may obtain the certificate in person, by mail or Online with credit card.

In person, proper identification is required, (State picture ID, or a valid drivers license).

By mail, you may use the request form provided here BIRTH(English) / BIRTH(Español) or DEATH (English. Money Orders or Cashier's Checks may be made payable to Osceola County Health Department. Furthermore, there is an additional $12.00 processing charge for all credit card orders.
Only parents, legal guardians, or the person whose name appears on the certificate, if they are at least 18 years of age, can receive a birth certificate. Complete all applications and provide a self address stamp 9 1/2 x 12 1/2 envelope and mailed to:

Florida birth certificates are available from 1930 to the present. Cost for birth certificates is:

$27.00 for each Photocopy birth certificates (Only for Osceola County Birth). $20.00 for the first certified computer copy and $8.00 for any additional computer birth certificates for that SAME person. Protect your Birth Certificates with Protective Covers for $3.00 each.

Death certificates for Osceola County may be purchased at $10.00 per certified copy for the years 1914 to present. Certain restrictions apply regarding who may receive a death certificate with cause of death listed
