Vital Statistics

New fees are as follow:

  • Birth Certificates Package- $20.00 *Includes protective cover
  • Birth Certificates - $17.00
  • Additional Birth Certificates - $8.00
  • Death Certificates - $10.00

Birth and Death Certificates are available through the Osceola County Health Department Vital Statistics Office and through the Osceola Regional Medical Center Office of Vital Statistics.

To Obtain Birth and Death Certificates:

You may obtain the certificate in person, by mail, by fax or Online with credit card.

  • To order ONLINE and obtain birth certificates go to www.vitalchek.com

  • To order ONLINE and obtain death certificates go to www.vitalchek.com

  • In person, proper identification is required, (State picture ID, or a valid drivers license).

  • By mail, you may use the request form provided here BIRTH(English) / BIRTH(Español) pdf file or DEATH (English) / Death (Español)pdf file.  The request must be notarized before it is forwarded to our office.  Money Orders or Cashier's Checks may be made payable to Osceola County Health Department.  Personal checks may delay the processing time.

  • You may order a birth or death certificate by using the on line forms provided here BIRTH(English) / BIRTH(Español) pdf file or DEATH (English) / Death (Español)pdf file.   and Fax it to our office.  Please ensure you provide the contact information so we can better serve you in the completing and ordering process. To order by Fax dial (407) 343-2084.  All fax orders must be paid for by credit card.  Furthermore, there is an additional $12.00 processing charge for all credit card orders.

Only parents, legal guardians, or the person whose name appears on the certificate, if they are at least 18 years of age, can receive a birth certificate.  The Vital Statistics office is open Monday through Friday from 8 a.m. to 4:30 p.m.  Completed applications may be mailed to:

Florida birth certificates are available from 1930 to the present.  Cost for birth certificates is:

  • $17.00 for each Birth Certificates.  $8.00 for any additional birth certificates. Protect your Birth Certificates with Protective Covers for $3.00 each.

  • Death certificates for Osceola County may be purchased at $10.00 per certified copy for the years 1914 to present.  Certain restrictions apply regarding who may receive a death certificate with cause of death listed
